The posture that decides what needs covering
Start with what a surfing body actually presents to the sky, because it is not the same as a standing body.
Paddling puts you prone with your head up. The surfaces facing upward are the back of the neck, the upper back and shoulders, the backs of the upper arms and, when you reach forward, the tops of the forearms and the backs of the hands. Sitting puts the tops of the shoulders, the back of the neck, the tops of the thighs and the tops of the feet upward. Standing on a wave is a small fraction of the session.
Add the reflected component from the water and you also have the underside of the chin, the jaw and the inner forearms receiving light. Which means the useful area to cover is more or less everything from the wrists to the neck, and that is precisely what a long sleeved top with a high neck does.
The three ways a top fails to cover what it should
It rides up at the waist. Paddling, popping up and the friction of a board deck all pull a top upward. The lower back is a well documented burn area in this sport and the reason is mechanical rather than a failure of attention. A longer cut, or a top designed to be worn tucked, addresses it.
The neck is cut too low. Most tops finish somewhere near the collarbone, which leaves the back of the neck and the throat open. Given that the back of the neck is the most reliably damaged area in the sport, that is the cut to check before anything else. See the back of the neck.
The sleeves stop short. A three quarter sleeve leaves the forearm exposed, and the forearm is the part reaching forward into the sun on every stroke. Full length to the wrist is not a marginal upgrade here.
Short sleeve against long sleeve
Most people who own one top own a short sleeved one, because it is cooler and it is what gets sold.
The arms are the largest area of skin a surfer leaves out in warm water. Both forearms, both upper arms, facing the sky through the whole paddle. Moving from short to long sleeves is probably the single largest reduction in exposed area available to a surfer for the price of one garment.
The comfort objection is real and worth handling directly rather than dismissing. Wet fabric in moving water is not the same as a dry shirt in a heatwave, and many people find a wet long sleeved top cooler than bare skin under direct sun once they are used to it. That is not universal and it depends on the fabric and the conditions. It is worth trying properly before deciding.

Fit is a protection question, not just a comfort one
Three fit properties change what the garment does.
Too loose. Drags in the water, catches on the board, rides up more, and irritates enough that it stops being worn.
Too tight. Permanently stretched fabric is more open fabric, particularly across the shoulders where it is under most tension and where the sun is landing hardest.
Right. Close enough not to drag, relaxed enough not to be taut, long enough in the body and the sleeve to stay where it was put.
The other fit variable nobody mentions is the chafing question. Any top worn under or without a wetsuit will chafe somewhere if the seams sit badly, and a garment that gives you a raw neck after two hours is a garment that will not be worn again. Flat seams and a neck that does not sit exactly where a wetsuit collar sits are worth checking.
Wearing one under a wetsuit
Many people wear a thin top under a suit for warmth or to stop chafing. For sun purposes it changes little, because the suit is already covering the area, but it has one relevant side effect: it makes the top a permanent part of the kit bag rather than a summer only item, which means it is there in spring when the suit gets shorter and the exposure question returns.
The transition point in spring is the one to watch. The week you move from a full suit to a shorty or a spring suit is the week your exposed area expands, and it is frequently the week the index is climbing fastest. That interaction is covered in the wetsuit gap.
Legs, and why nobody covers them
The tops of the thighs face the sky whenever you sit on a board, which is most of a session. In boardshorts they are uncovered, and the thighs are a large area.
Longer boardshorts, surf leggings and full length neoprene all cover them, and all of them exist as categories. Whether you use any of them is a personal call, and this manual is not going to pretend that a surfer in warm water is likely to put leggings on. What is worth knowing is simply that the thighs are exposed for the whole session and that most people have never once thought about it, which makes the burn on the tops of the legs after a hot day a genuine surprise every single time.
What to take away
A paddling body presents the back of the neck, the shoulders, the backs of the arms, the forearms and the tops of the thighs to the sky for hours. A long sleeved top with a high neck covers most of that and does so for the entire session.
Buy for sleeve length, neck height and construction, get the fit right so it stays where you put it and does not chafe, and take the short to long sleeve upgrade seriously, because it is the biggest single reduction in exposed area available for one purchase.
