Why lips are a category of their own
Every other area in this section is a coverage problem with a garment answer. Lips are not. There is no garment. There is no headwear that covers them while you surf. They are permanently, unavoidably exposed for the whole session.
They are also different biologically. The vermilion of the lip is thin, and it does not have the same pigment defence that the rest of your skin has. That is a general fact about lips, not a surfing one, and it means the same exposure lands harder there.
Then add the surf specific part, which is where it gets difficult.
The lower lip takes the reflected light
This is the detail that makes lips a surfing subject rather than a general one.
On land, the upper lip and the area under the nose are somewhat shaded by the nose itself, and the lower lip sits below the mouth in relative shadow. Light comes down and the geometry of the face provides some shelter.
On water, light comes up. The lower lip, which faces slightly downward and outward, is directly in the path of the reflected component. So is the strip between the nose and the upper lip, and the underside of the chin. Those are areas that on land would be considered self shading, and on the water they are not.
Which is why a surfer's lip problem tends to concentrate on the lower lip specifically, and why it looks different from a hillwalker's, whose upper lip and nose take more of it.
Why nothing stays on them
List the mechanisms acting on your lips during a session and the situation becomes clear.
They are permanently wet. Salt water, spray and immersion.
You lick them. Constantly, involuntarily, and more when they are dry, which is a cycle that makes it worse.
Salt dries on them. And salt is drying and abrasive, which makes them chap, which makes you lick them more.
Wind works on them. Wind and evaporation are as hard on lips as anything, which is why a windy session leaves them raw.
You wipe your face. Every time you clear your eyes, your hand crosses your mouth.
You cannot reapply. Which is the entire problem of this sport, applied to the one area that most needs reapplying.

What actually helps
Being straight about it: this is the area where the available answers are weakest, and pretending otherwise would be useless.
Apply something before you go out. Lip products with sun protection exist as a category. This manual names none of them. Applying before entry is the one opportunity you get, and it is better than nothing even though it will not last the session.
Reapply if you genuinely come in. Between sessions, at the car park, whenever there is a real break. Unlike face application this one is quick, does not require clean hands to the same degree, and is realistic.
Keep one where you will use it. In the van, in the wetsuit bag, in the pocket of the jacket you wear to the beach. A lip product at home protects nobody. This is the single most effective intervention available and it costs nothing but organisation.
Stop licking them. Easier said than done, and worth trying, because it is genuinely part of the cycle.
Shorten the exposure by other means. Since you cannot cover lips, the timing lever matters more here than anywhere. A session at a lower sun angle reduces what arrives at the one area you cannot protect. See the hours that matter.
Chapping, which is a different problem
Lips after a long windy session are often raw, cracked and sore, and that is largely a wind, salt and dehydration matter rather than an ultraviolet one. The NHS publishes practical material on dry and chapped lips and it is the sensible place to read about it.
The two problems interact, though. Chapped lips get licked more, hold anything applied less well and are more uncomfortable, which makes people less likely to bother with the routine at all. So dealing with the chapping is indirectly part of dealing with the exposure.
Drinking properly matters here too, and is dealt with in hydration, heat and performance.
Where lips go clinical
This is a point where this manual stops firmly. Lips are an area where persistent changes matter and where people are inclined to ignore things because a sore lip after a windy session is normal.
A sore, a crack, a rough patch or an ulcer on the lip that does not heal, that keeps coming back in the same place, or that is changing, is a reason to see a GP or a dentist. Not because a surf website can tell you what it is, but because that is precisely the sort of thing that gets dismissed as chapping for months. The NHS is the right starting point and an appointment is the right action.
The general rule this manual uses throughout: anything not healing gets looked at by somebody qualified. That is the whole of the advice and it is deliberately the whole of it.
What to take away
Lips have no cover option, little defence of their own, and receive light from above and reflected from below, with the lower lip taking the reflected component. Nothing stays on them because they are wet, licked, salted, wind dried and wiped all session.
Apply before entry, keep something in the van rather than at home so that the between session reapplication actually happens, and lean harder on timing here than anywhere else, because it is the one area you cannot cover.
